What is 5-HIAA, Urine Random?
5-Hydroxyindoleacetic acid (5-HIAA) is the primary breakdown product of serotonin. This test measures 5-HIAA levels in a single (random) urine sample, usually expressed as a ratio to urine creatinine.
Serotonin is produced from the amino acid tryptophan and is then broken down by monoamine oxidase into 5-HIAA, mainly in the liver. Because 5-HIAA is excreted in the urine, measuring it provides an indirect way to assess serotonin production in the body.
A random urine sample is more convenient than the standard 24-hour collection, though it may be less sensitive for detecting intermittent elevations.
Where Does 5-HIAA, Urine Random Come From?
Serotonin is produced primarily by enterochromaffin cells in the gastrointestinal tract (~90%) and in smaller amounts by neurons in the brain. It is then metabolized to 5-HIAA mainly in the liver by monoamine oxidase A, after which 5-HIAA is excreted by the kidneys.
