What is Acetoacetic Acid?
Acetoacetic acid (also called acetoacetate) is one of three ketone bodies produced by the liver when the body breaks down fat for energy. The other two ketone bodies are beta-hydroxybutyrate and acetone.
When glucose is in short supply, such as during fasting, prolonged exercise, or uncontrolled diabetes, the liver converts fatty acids into ketone bodies as an alternative fuel. Acetoacetate accounts for roughly 20% of circulating ketones, while beta-hydroxybutyrate makes up about 78% and acetone about 2%.
Low levels of acetoacetate are normal and reflect routine fat metabolism. Significantly elevated levels signal that the body is relying heavily on fat for fuel, which can indicate a metabolic emergency such as diabetic ketoacidosis.
Where Does Acetoacetic Acid Come From?
Acetoacetic acid is produced in the liver through beta-oxidation of fatty acids. When insulin is low and glucagon is high, the liver converts fatty acids into acetyl-CoA, which is then condensed into acetoacetate and released into the bloodstream for use by other tissues.
