Creatine is a naturally occurring compound found mainly in skeletal muscle, where it stores and transports energy as phosphocreatine. It is not the same as creatinine, which is the waste product formed when creatine breaks down, or creatine kinase, the enzyme that transfers a phosphate group to creatine.

A serum creatine test measures the amount of creatine circulating in your blood. About 95% of the body's creatine is stored in skeletal muscle, with smaller amounts in the brain and heart.

Because most creatine is inside muscle cells, elevated blood levels can indicate that creatine is leaking out of damaged or diseased muscle tissue.

Creatine is synthesized in a two-step process: the kidneys produce an intermediate called guanidinoacetate (GAA), which the liver then converts into creatine. Creatine also comes from dietary sources, mainly meat and fish.
