What is Free Dihydrotestosterone?
Free dihydrotestosterone (free DHT) is the small fraction of DHT in your blood that is not bound to carrier proteins. This unbound portion is immediately available to bind androgen receptors in target tissues, making it the biologically active form.
DHT is the most potent natural androgen, with roughly double the binding affinity for the androgen receptor compared to testosterone and a much slower dissociation rate. It is produced from testosterone by the enzyme 5-alpha reductase, an irreversible conversion. Circulating DHT is approximately 10% of circulating testosterone concentration.
Only about 1% of circulating DHT is free. Roughly 60% is tightly bound to SHBG and 39% is loosely bound to albumin. Because DHT binds SHBG with about five times the affinity of testosterone, a smaller share circulates in the free form compared to testosterone.
Where Does Free Dihydrotestosterone Come From?
DHT is produced primarily in peripheral target tissues (prostate, skin, hair follicles, liver, genital skin) through conversion of testosterone by 5-alpha reductase. The testes (men), ovaries (women), and adrenal glands supply the testosterone and androgen precursors used as substrate.
