Cholic acid is one of the two primary bile acids your liver makes from cholesterol. The other is chenodeoxycholic acid (CDCA). Together with their conjugated and secondary forms, they make up over 95% of the bile acid pool in your body.

Cholic acid itself accounts for roughly 31% of human bile. Before being secreted into the intestine, the liver conjugates it with glycine or taurine to improve its solubility.

The serum cholic acid test measures the small fraction that spills from portal blood into general circulation. Elevated fasting levels suggest the liver is not clearing bile acids properly, making this a sensitive indicator of hepatobiliary dysfunction.

Synthesized in the liver from cholesterol through a multi-step enzymatic pathway. The enzyme CYP8B1 adds a hydroxyl group that commits the pathway to producing cholic acid rather than chenodeoxycholic acid. About 95% of secreted bile acids are reabsorbed in the terminal ileum and recycled back to the liver through enterohepatic circulation.
