What is Immature Reticulocyte Fraction?
The Immature Reticulocyte Fraction (IRF) measures the proportion of the youngest, least mature reticulocytes circulating in your blood. Reticulocytes are newly formed red blood cells that still contain residual RNA from their development in the bone marrow. IRF isolates the fraction with the most RNA, meaning they were most recently released.
It is expressed as a percentage of total reticulocytes. A standard reticulocyte count tells you how many new red blood cells the marrow is producing; IRF tells you whether that production is accelerating or slowing down.
Normal IRF ranges are approximately 3.0–15.9% for men and 2.3–13.4% for women. It is typically reported as part of a reticulocyte profile alongside the absolute reticulocyte count and reticulocyte hemoglobin content.
Where Does Immature Reticulocyte Fraction Come From?
Reticulocytes originate in the bone marrow, where red blood cells mature through several stages before being released into the bloodstream. Under normal conditions, reticulocytes spend one to two days maturing in the marrow before release. When the body needs more red blood cells, erythropoietin (produced primarily by the kidneys) signals the marrow to release reticulocytes earlier, while they still contain more RNA, which raises the IRF.
