What is 11-Deoxycortisol?
11-Deoxycortisol (also called Compound S) is a steroid hormone and the immediate precursor to cortisol. It is produced in the adrenal glands as part of the glucocorticoid synthesis pathway.
The enzyme 11-beta-hydroxylase (CYP11B1) converts 11-deoxycortisol into cortisol in the final step of cortisol production. When this enzyme is blocked or deficient, 11-deoxycortisol accumulates in the blood.
On its own, 11-deoxycortisol has very little hormonal activity. Its main clinical value is as a diagnostic marker, particularly in the metyrapone stimulation test used to evaluate the body's cortisol-producing axis.
Where Does 11-Deoxycortisol Come From?
Produced in the zona fasciculata of the adrenal cortex, the same layer responsible for cortisol synthesis. Its production is driven by ACTH from the pituitary gland.
