What is 2-Methoxyestradiol, Urine Creatinine Ratio?

2-Methoxyestradiol (2-MeO-E2) is a metabolite of estradiol formed during Phase II estrogen detoxification. It is produced when the catechol estrogen 2-hydroxyestradiol (2-OH-E2) is methylated by the enzyme catechol-O-methyltransferase (COMT), using S-adenosylmethionine (SAMe) as a methyl donor.

This marker measures 2-MeO-E2 in a urine sample, divided by urinary creatinine concentration to correct for urine dilution. The ratio provides a standardized estimate of daily metabolite output from a single spot sample.

Unlike its parent estrogens, 2-MeO-E2 has no significant estrogenic activity. Instead, it has distinct antiproliferative and anti-angiogenic properties. Measuring this metabolite helps assess how effectively the body methylates and deactivates catechol estrogens through the COMT pathway.
