2-Methoxyestrone (2-MeO-E1) is a metabolite of estrone, one of the body's main estrogens. It is formed when the enzyme catechol-O-methyltransferase (COMT) adds a methyl group to 2-hydroxyestrone, converting it into a less active and potentially protective compound.

This marker has very low affinity for estrogen receptors and lacks significant estrogenic activity. It reflects the final step of the protective 2-hydroxylation pathway of estrogen metabolism.

The result is expressed as a ratio to urine creatinine to account for differences in urine concentration between samples.

2-Methoxyestrone is produced throughout the body when the COMT enzyme methylates 2-hydroxyestrone. The parent estrogen, estrone, is made primarily in the ovaries (premenopausal women), adipose tissue, and adrenal glands.
