What is Corticosteroid-Binding Globulin?
Corticosteroid-binding globulin (CBG), also called transcortin, is a protein made by the liver that carries cortisol through the bloodstream. It belongs to the serpin family of proteins and is encoded by the SERPINA6 gene.
About 80-90% of cortisol in the blood is bound to CBG, with another 7-15% loosely bound to albumin. Only about 5% circulates as free cortisol, which is the biologically active form that cells can use.
Because CBG determines how much cortisol is bound versus free, its levels directly affect the interpretation of total cortisol results. A change in CBG can make total cortisol appear high or low even when free cortisol is normal.
Where Does Corticosteroid-Binding Globulin Come From?
CBG is produced primarily by the liver. Small amounts are also made in the kidneys, lungs, adrenal glands, and other tissues, where it may help regulate local cortisol availability.
