Guanosine monophosphate (GMP) is a purine nucleotide, one of the four basic building blocks of RNA. It consists of the nucleobase guanine, the sugar ribose, and a phosphate group.

In blood, GMP normally circulates at very low concentrations because cells rapidly use it or break it down. Measuring GMP reflects the balance between purine production and purine breakdown in the body.

Elevated GMP can signal accelerated cell turnover, enzymatic defects in purine metabolism, or increased immune activity. It is typically measured as part of a specialized purine metabolite panel rather than routine blood work.

GMP is produced in all cells through two pathways: de novo synthesis (building it from scratch, primarily in the liver and rapidly dividing cells like bone marrow, gut lining, and immune cells) and the salvage pathway, which recycles the purine base guanine back into GMP.
