4-Methoxyestrone (4-MeO-E1) is a methylated estrogen metabolite produced when the enzyme COMT (catechol-O-methyltransferase) adds a methyl group to 4-hydroxyestrone (4-OH-E1). This methylation step is part of Phase II estrogen detoxification and converts a potentially harmful catechol estrogen into a safe, inactive end product.
The metabolic pathway runs: estrone → 4-OH-estrone (via CYP1B1, Phase I) → 4-MeO-estrone (via COMT, Phase II). Because unmethylated 4-OH-E1 can oxidize into reactive quinones that damage DNA, this methylation step is considered protective.
The urine creatinine ratio normalizes 4-MeO-E1 concentration against urinary creatinine to account for differences in hydration, providing a consistent measure of excretion regardless of how dilute or concentrated the urine sample is.

4-MeO-E1 is produced primarily in the liver, where COMT methylates 4-OH-E1 using S-adenosylmethionine (SAMe) as the methyl donor and magnesium as a cofactor. COMT is also active in the kidneys, brain, red blood cells, and hormone-sensitive tissues such as the breast and uterus.
