5α-Dihydrotestosterone (5a-DHT) is the most potent naturally occurring androgen in the body. It is formed when the enzyme 5-alpha reductase converts testosterone into its more active form in target tissues such as the prostate, skin, and hair follicles.

This test measures urinary 5a-DHT normalized to creatinine, which corrects for differences in hydration and urine concentration. It reflects the body's overall 5-alpha reductase activity and androgen metabolism rather than a single-point blood measurement.

5a-DHT has approximately three times greater affinity for androgen receptors than testosterone, making it the primary driver of androgen-dependent effects in many tissues.

5a-DHT is produced locally in target tissues (prostate, skin, hair follicles, and genital tissue) when the enzyme 5-alpha reductase converts circulating testosterone. The testes, ovaries, and adrenal glands produce the testosterone precursor.
