What is Deoxycorticosterone, Urine Creatinine Ratio?

Deoxycorticosterone (DOC) is a steroid hormone produced by the adrenal glands that acts as a mineralocorticoid, meaning it helps regulate the balance of sodium and potassium in the body. It is a precursor in the pathway that produces aldosterone, the body's primary mineralocorticoid.

This test measures DOC levels in urine, corrected for creatinine concentration. Creatinine correction accounts for differences in urine dilution, providing a more consistent and reliable result than a raw urine measurement alone.

Although DOC has only about 1/20th the sodium-retaining potency of aldosterone, it can still have significant clinical effects when levels are elevated.

Where Does Deoxycorticosterone, Urine Creatinine Ratio Come From?

DOC is produced primarily in the zona fasciculata of the adrenal cortex. It is synthesized from progesterone by the enzyme 21-hydroxylase and is then normally converted to corticosterone by 11β-hydroxylase on its way to becoming aldosterone.
