Etiocholanolone is a steroid hormone metabolite produced when your body breaks down DHEA and testosterone. It is measured in urine and corrected for creatinine to account for differences in urine concentration.

Etiocholanolone is formed through the 5β-reductase pathway. Testosterone is first converted to 5β-dihydrotestosterone by the enzyme 5β-reductase (primarily in the liver), and then further reduced to etiocholanolone. Because of this, etiocholanolone levels reflect how actively your body is using the 5β (non-androgenic) metabolic route.

Together with androsterone and DHEA-S, etiocholanolone helps estimate total DHEA production from the adrenal glands. The ratio of androsterone to etiocholanolone reveals whether your metabolism favors the more androgenic 5α pathway or the less androgenic 5β pathway.
