Waist circumference is a simple tape-measure assessment of abdominal girth, used as a clinical proxy for visceral fat, the metabolically active fat stored around organs deep in the abdomen.

It is typically measured at the midpoint between the lower margin of the last palpable rib and the top of the iliac crest (hip bone). Because visceral fat cannot be seen or felt directly, waist circumference offers a practical, low-cost estimate of central adiposity.

It is one of the core criteria for diagnosing metabolic syndrome and is considered a stronger predictor of cardiometabolic risk than BMI alone, because BMI cannot distinguish between fat and lean mass or between visceral and subcutaneous fat.
